Why Is a Geriatric Care Plan Important?

Are you an older adult or responsible for caring for someone who is? If yes, a geriatric care plan is crucial for you.

Geriatric care plans completed by a registered nurse are person-centered, holistic, and include the client’s wishes with a goal to improve care and achieve optimal health and wellness. Nursing care plans consider all diagnoses, level of functioning, mental health status, medications, the environment, and the client’s support system. A home visit is completed wherever home is; independent living, assisted living, or memory care facility.

You may not realize it, but aging is a risk factor for various medical conditions, from physical to psychological. A geriatric care plan is a crucial part of planning for future health. It provides a road map for what older adults must do to stay healthy. The plan ensures you or your loved one is well-informed about medical requirements, and it will provide education about medical diagnosis, medications, and treatment options.

What's Included?

Initial visit:

A registered nurse completes a 2-hour comprehensive assessment and environmental screening, with a holistic approach. A holistic nursing assessment focuses on more than one physical condition. It evaluates psychological, spiritual, and social status as well because these factors are all intertwined to make up a whole person.

A physical exam with vital signs is completed and medical history is obtained so there is a clear understanding of the current and past health conditions. This identifies needs and risks correctly. Once all the information is gathered, a plan with detailed instructions is generated.

This geriatric care plan may include instructions such as following up with a specialist to address a diagnosis, dietary changes, incorporating exercise, or making environmental changes to allow one to age in place safely. The client’s support system is informed of the plan because a team approach achieves better outcomes.

Follow-up visits:

A registered nurse completes a 1-hour visit with a focused nursing assessment, environmental screening, and review of the care plan. The care plan may be revised based on needs and risks.
